ICU Form V: PHOTO

You must submit 2 photos pasted to ICU Form II and V.
These photos must be identical passport-style photographs taken recently (4 cm in height × 3cm in width, with your name and application number written on the back).
Photographs must be taken against a plain background. No headgear may be worn in the photograph.
The quality of the photos should be VERY GOOD.
Photocopied pictures or photos printed with a home (personal) printer are NOT acceptable.
The 2 photos will be used in producing your student ID card and for other office use.
