ICU Form II: UNIVERSITY MASTER RECORD & PERSONAL DATA SHEET

All Accepted Applicants must submit this form with a photo attached as explained below.
Please write your name in Roman alphabets as it appears on your passport, and if applicable, your name in kanji as it appears on family register(koseki 戸籍).

Please select one of the following statuses and fill it in “Status at ICU” item.
CLA Regular / CLA Transfer / CLA One-Year-Regular / CLA Kenkyusei

Photograph

Passport-style photograph taken recently (4 cm in height ×3cm in width, with your name and application number written on the back) should be glued to the form.
Photograph must be taken against a plain background. No headgear may be worn in the photograph.
The quality of the photo should be VERY GOOD.
Photocopied pictures or photos printed with a home (personal) printer are NOT acceptable.

Name to be Registered at ICU

The prescribed forms mentioned below are available at the Educational Affairs Group (EAG).

A. NAMES WRITTEN IN KANJI

For students with Japanese citizenship:
The name in kanji shown on university materials such as ID cards, graduation certificates, and other official certificates issued by the university during a student's enrollment and after graduation will be the same as that of the student's residential registry (jumin-kihon-daicho 住民基本台帳). These are also the same kanji as shown in the student's family registry (koseki 戸籍). However, for internal university materials (internally distributed and sent documents) kanji used must conform to the JIS (Japanese Industrial Standards) tiers 1 and 2.

For students with non-Japanese citizenship:
The name will be written in alphabetical characters. If a student would like his/her commonly used name as appeared in the residential registry (jumin-kihon-daicho 住民基本台帳) or on his/her residence card to be used, then the same rules apply as those for students with Japanese citizenship.

B. NAMES WRITTEN IN ALPHABETICAL CHARACTERS

For students with Japanese citizenship:
It is the university's principle to use the Hepburn system of Romanization (ヘボン式). If a student has a passport, then the name on his/her passport will be used, except a name indicated in parentheses, which will be omitted.

For students with non-Japanese citizenship:
Names will be shown as they are on student's passports and residence card (zairyu-kado 在留カード). If a student's commonly used name as it is in the residential registry (jumin-kihon-daicho 住民基本台帳) is to be used, then this will follow the Hepburn system of Romanization.
N.B.1 : For students with multiple citizenship:
①Multiple citizenship with Japan: It is possible to register the name of only one nationality.

②Multiple citizenship of other than Japan: It is possible to register the name of one nationality whose passport being used to enter Japan.
    Please change the nationality of student registry when he/she changes the nationality of the residence card (zairyu-kado 在留カード).
If your name is changed, please change your name as well.


N.B.2 : A student can change his/her name (for example, from commonly used name to proper name, vice versa, or in the case of multiple citizenship, from the name of one nationality to another) only once during the enrollment.

C. STUDENT'S MAIDEN NAME REGISTERED AT THE UNIVERSITY

It is the university's principle to use the same name as shown in the student's family registry (koseki 戸籍) for Japanese students, and on passport and residence card (zairyu-kado 在留カード) for International students (hereafter referred to as "official legal name"). In addition, student's maiden name may be registered at the university.
However, the university policy that the only one name can be registered has not been changed, so students should be well aware that if a need arises to certify the university-registered name is different from the official legal name, the university is unable to certify since it is not authorized to do so, and students themselves owe responsibility for such cases.
Also, if procedures involve organizations outside the university, such as application for scholarship, employment as part-time teaching assistant or class helper, student name will be treated according to teh rules of the university offices concerned.
【Procedure】
Fill out a prescribed form and submit it to the Educational Affairs Group with an official document that certifies the official legal name and maiden name.
Students can change their name only once during their enrollment, including the entrance procedure.
