Please read each page carefully and submit/send in the nessary documents according to your status by the due date.

Please refer to STEP II as the required forms differ depending on your status.
Forms can be downloaded in STEP II. Please check the steps for each status, and print both sides on A4 size blank paper.

• ICU Form I: GOOD CONDUCT SURETY
• ICU Form II: UNIVERSITY MASTER RECORD PERSONAL DATA SHEET
• ICU Form III: HEALTH INFORMATION FORM
• ICU Form IV: 住民基本台帳記載事項証明書（証明願）
• ICU Form V: PHOTO
• ICU Form VI: QUESTIONNAIRE CONCERNING ENGLISH FOR LIBERAL ARTS (ELA) (Applicable for the Gokakusha admitted by "EJU利用選抜（9月入学）")
• COPY OF PASSPORT
• COPY OF THE PROOF OF BANK TRANSFER OF FEES
